Model cancellation form

For the 14-day right to cancel in section 8 of our Terms of Service

You don’t have to use this form. Any clear message is enough, for example an email to billing@octodecks.com saying you want to cancel. To stop your subscription renewing at any time (not just in the first 14 days), use the Stripe billing portal.

Complete and return this form only if you wish to withdraw from the contract. Copy the text into an email to billing@octodecks.com, or post it to the address below.

What happens next is explained in section 8 of our Terms: we refund you within 14 days, less a proportionate amount for any days of service you used.
